CDC Hourly Care Orientation - Preparation

What should I bring?

Preparing for an exciting and fun filled day at hourly care.


Hourly Care Packet

Please remember to bring your child's complete hourly care packet. For first time hourly care users, this packet will be supplied to you at your child's first hourly care appointment.

This packet will include the following important information:

  • Program Agreement
  • Medical Consent Form
  • Health Assessment
  • Shot Record
  • Multi-Disciplinary Inclusion Action Team (MIAT) paperwork (MIAT Tool#1 and #2, if applicable, and appropriate Medical Action Plans and/or Special Diet Statement)


Clothing
One complete extra change of clothes (i.e., socks, pants, shirt, underwear)


Diapers/Pull-Ups/Wipes
For children that wear diapers or are potty-training and wearing training pants, one diaper/pull-up is required for every hour of child care. For example, if your child is attending hourly care for 3 hours, 3 diapers or pull-ups must be brought in with you. Please ensure diapers are individually labeled, as well as wipes.


Bottles
For infants, 12 months and younger, prepare bottles prior to bringing them to the CDC.
Bottles must:

  • Be non-glass, or glass with a silicone sleeve, clean and labeled with the, (1) child's legal first and last name, (2) date, and (3) contents of bottle (i.e., breastmilk, Similac)
  • Have a cap labeled with the child's name

The CDC provides the following infant formulas:

  • Similac Advance with Iron
  • Isomil Advance with Iron

If your child is fed a different formula, the parent must supply the formula already mixed and prepared in individually labeled bottles.

Parents are required to bring one bottle for every 2 hours of child care.

Due to potential choking hazard, infant cereal is not permitted in bottles.

Do not bring sippy cups – sippy cups are provided by the CDC.


Medication
Any medication or basic care item must be on the CYS Approved Medication List to be administered during child care.

If your child is prescribed any type of rescue medication such as Albuterol, Xopenex, Epi-Pen Jr., the medication must be brought each time and kept on-site during your child's visit. Any equipment required to administer the medication such as a nebulizer must also be brought.

Medication must be labeled with the following:

  • Child's legal first and last name
  • Health Care Provider's name
  • Pharmacy phone number
  • Name of medication
  • Dose and specified time of administration
  • How to apply, if applicable
  • Special instructions
  • Start/stop dates

For any medication, you will be asked to complete a DA Form 5225-R, Medication Dispensation Record, authorizing child care staff to administer the medication. Each card covers a period of 90-days.


Over the counter (OTC) basic care items are limited to

  • Skin creams: (i.e., A&D Ointment, Desitin, Balmex NF, petroleum jelly, zinc oxide, Eucerin)
  • Teething pain: Orajel (must be benzocaine free and cotton swabs must be supplied for application)
  • Lip medication: Lip balm, must be labeled "for child use" and all natural
  • Sunscreen: Must be approved and marketed for pediatric use, SPF 15 or greater. No aerosol sunscreen sprays are permitted.


Please check your child's diaper bag/backpack prior to leaving to ensure there are no other items left inside that are not required for care.
